Payment & Direct Billing Policy

Payment is due at the time of your appointment.

Urban Massage & Wellness is pleased to offer direct billing to many insurance providers as a courtesy to our clients. Direct billing is a convenience service and is not guaranteed.

Insurance coverage varies between providers and individual plans. It is the client’s responsibility to understand and verify their eligibility, coverage, referral requirements, deductibles, co-payments, annual maximums, and any other policy requirements or limitations. Urban Massage & Wellness does not have access to your complete insurance policy, remaining benefits, or coverage limits.

Third-Party Direct Billing Systems

Direct billing claims are submitted through systems operated by insurance companies and third-party providers. These systems are outside of the control of Urban Massage & Wellness and may occasionally be unavailable due to scheduled maintenance, technical issues, system outages, connectivity problems, or other circumstances beyond our control.

While we will make reasonable efforts to submit eligible claims electronically, we cannot guarantee that direct billing will be available or successfully processed at the time of your appointment.

If we are unable to submit your claim electronically for any reason, payment in full is required at the time of your appointment. You will be provided with the appropriate receipt so that you may submit your claim directly to your insurance provider for reimbursement, if eligible.

The availability of direct billing does not affect the status of your scheduled appointment. An inability to process a direct billing claim does not constitute a cancellation of your appointment by Urban Massage & Wellness.

If you choose not to proceed with your scheduled appointment because direct billing is unavailable, our regular cancellation policy may apply.

Insurance Claims & Outstanding Balances

Direct billing does not guarantee that your insurance provider will approve or pay your claim.

If a claim is declined, partially paid, delayed, remains pending, requires additional information, or is otherwise not paid in full by your insurer, you remain responsible for any outstanding balance.

A valid credit card is required to be securely maintained on file for all appointments and may be charged in accordance with our clinic policies when a balance remains outstanding.

We encourage all clients to confirm their insurance coverage directly with their provider before their appointment if they have questions or if their ability to attend depends on insurance reimbursement.
